What is In-Vitro Fertilization?

IVF is a medical procedure whereby a female egg is fertilized by male sperm in a test tube or elsewhere outside the body under the care of fertility specialists (Embryologist, Gynaecologist, etc). In-Vitro means outside the body.

What is the process of In-Vitro Fertilization (IVF) treatments?

During the process of IVF, initially, the ovaries are inducted and stimulated using fertility drugs for boosting and production of multiple eggs than normal single production. The process begins at the initial menstruation cycle after stimulation. 

The mature eggs are then collected (retrieved) from the ovaries and allowed to be fertilized by sperm in a laboratory under the culture medium.   1. Intrauterine Insemination (IUI) is one of the alternatives before IVF decisions. Let’s discuss…

Intrauterine insemination is a procedure in which treated sperm is injected into a woman’s uterus directly at the time of ovulation, to improve the likelihood of fertilization. This procedure is possible with the use of fresh sperm (from the partner) or frozen sperm (from the partner or a donor). The success rate may not be markedly higher than that achieved with timed sexual intercourse naturally but it is worth trying rather than not conceiving, our doctors can predict its viability based on your conditions.

Intrauterine insemination treatment options include:

Natural cycle insemination: This doesn’t involve any medication and follows the woman’s natural menstrual cycle. It may be used for patients who are unable to conceive with natural intercourse. It’s not usually successful for women whose partners have poor sperm quality or in women older than 35 years.

Ovulation induction with clomiphene citrate: This involves the use of medication (Clomid or Seraphine) to stimulate the ovaries and encourage or regulate ovulation. Using this medication in conjunction with intrauterine insemination has been shown to increase pregnancy rates over natural cycle insemination.

Ovulation induction with follicle-stimulating hormone (FSH): Ovarian stimulation is stronger with this method and it has been shown to increase pregnancy rates over natural cycle insemination and clomiphene citrate ovulation induction. We are very careful to regularly monitor our patients to minimize possible complications relating to overstimulation and reduce the risk of multiple pregnancies.

It is worth noting that studies suggest that the most successful pregnancy results from intrauterine insemination are achieved using clomiphene citrate or follicle-stimulating hormone (FSH) treatments. With any IUI, a “trigger” injection may be needed the day before.

  1. Hormonal Therapy for men

Hormonal imbalances that affect sperm development may be treated by gonadotrophin therapy. Gonadotropins are protein hormones sometimes used to treat unexplained male infertility in the cases of abnormally low sperm counts or when less than 40% of sperm are mobile. Other male fertility drug treatments can include using antibiotics to treat infertility caused by infections.

  1. Surgery

For women, surgery can often improve chances of conception when the cause of infertility can be traced to past inflammation or infections that have created scarring, or conditions such as fibroids, endometriosis, and other tubal or uterine issues. For men, a condition known as varicocele, where the veins of the scrotum become abnormally enlarged, may be surgically treated by clipping or tying the veins.

To reverse sterilization treatments such as tubal ligation and vasectomy, surgery requiring an operating microscope, known as microsurgery, can be used. What is intracytoplasmic sperm injection (ICSI)?

In General normal morphology of the sperm, before a man’s sperm can fertilize a woman’s egg, the head of the sperm must attach to the outside of the egg membrane. Once attached, the sperm pushes through the outer layer to the inside of the egg (cytoplasm), where fertilization takes place.

Some men’s sperm are abnormal morphologically, in such conditions the sperm cannot penetrate the outer layer of the egg, for a variety of reasons. The egg’s outer layer may be thick or hard to penetrate or the sperm may be unable to swim or weak. In these cases, a procedure called intracytoplasmic sperm injection (ICSI) plays an alternate option for penetration of the egg to fertilize. ICSI is done along with in vitro fertilization (IVF) to help fertilize the egg. During ICSI, a single sperm is injected directly into the cytoplasm of the egg.

How does ICSI work?

There are two options that an egg may be fertilized by IVF techniques: 

  • Traditional and
  • ICSI. 

In traditional IVF, 50,000 or more swimming sperm are placed next to the egg in a laboratory dish. Fertilization occurs when one of the sperm enters into the cytoplasm of the egg. In the ICSI process, a tiny needle, called a micropipette, is used to inject a single sperm into the center of the egg. With either traditional IVF or ICSI, once fertilization occurs, the fertilized egg (now called an embryo) grows in a laboratory for 1 to 5 days before it is transferred to the woman’s uterus (womb).

What is Cryopreservation?

Cryopreservation is the process of cooling and storing cells (female eggs, male sperms), tissues, or organs at very low temperatures to maintain their viability for a long period. In this process, biological materials including cells, oocytes, spermatozoa, tissues, ovarian tissues, pre-implantation embryos, organs, etc. are kept in extremely cold temperatures without affecting the cell’s viability.

Dry Ice and liquid nitrogen are generally used in this method. For example, the technology of cooling and storing cells at a temperature below the freezing point (‘196’ C) permits high rates of survivability of the cells upon thawing.

The embryos, eggs, and sperms are preserved in the following complete procedure steps:

  • Harvesting of the cells
  • Addition of cryo-protectants agents such as glycerol, FBS, salts, sugars, glycols is added to the samples as it reduces the freezing point of the medium.
  • Freezing: Different methods of freezing are applied to protect cells from damage and cell death by their exposure to the warm solutions of cryoprotective agents.
  • Storage in liquid nitrogen: The prepared samples are stored in extreme cold or -80°C in a freezer for at least 5 to 24 hours before transferring it to the storage vessels.
  • Thawing: The process of warming the biological samples to control the rate of cooling and prevent the cell damage caused by the crystallization.

What is an assisted gametes Donation program?

Assisted gamete donation means using eggs, sperm, or embryos from someone else to help an intended parent(s)/infertile person to have a child via IVF treatments and surrogacy. 

Why would this be done?

Gamete or embryo donation makes it possible to have a child when one or both partners are not able to provide their sperm, eggs, or embryos. Egg or sperm donation allows one of the intended parents to keep the genetic link to the child. With egg donation, the intended parent can experience a biological connection to the child through the pregnancy. With embryo donation, there is no genetic link. However, the intended parents can experience pregnancy and birth.
