In Vitro Fertilization

In vitro fertilization (IVF), is a complex process that helps with fertility, prevents genetic problems, and helps in the conception of a baby. IVF involves the collection of mature & healthy eggs from the ovaries. Further, the eggs are then fertilized in a laboratory with sperm. The fertilized egg or eggs (embryos) are then transferred to a woman’s uterus for fertilization. A full IVF cycle gets completed in approximately three weeks. These steps can sometimes be simplified into smaller parts, which can make the process take longer than usual. IVF is the most efficient & effective form of assisted reproductive technology. You can use your eggs or your partner’s fertilized eggs. You may also use eggs, sperm, or embryos from an anonymous donor. A gestational carrier, which is a woman with an embryo implanted within her uterus, may be used in some cases. The chances of you having a healthy baby through IVF will depend on many factors such as your age and the reason for infertility. Multiple pregnancies can occur if more than one embryo is transferred into your uterus. IVF can sometimes be used as a primary treatment for infertility for women older than 40. If you have certain medical conditions, IVF may be possible.

Overview of IVF journey

Understanding “INFERTILITY”- Both men and women can experience infertility from a variety of causes. The most common causes for women are infertility due to lack of regular ovulation, damaged or blocked fallopian tubes, and known medical conditions like endometriosis, PCOS, or PCOD. Poor-quality semen is the leading cause of subfertility in men.

KEY FACTS

  • One in six couples have difficulty in conceiving.
  • Nearly 3.5 million people in the around the globe are affected by infertility
  • Over 40% of  are  serious male infertility issues
  • 15% of infertility cases are still unreported.

Process of IVF

The IVF treatment begins with hormone therapy that stimulates the development of follicles within the ovary. These eggs are then taken and fertilized to create multiple embryos. One or two embryos from each group are then transferred to the uterus through the vagina. This is where implantation takes place. Fertility World has broken down the IVF process into six easy stages to explain what happens during an IVF procedure.

Stage 1:

A course of medication is administered to stimulate the ovaries to produce eggs. You will be required to attend the clinic to undergo a series of monitoring scans in order to monitor the development of the follicles which produce eggs. After your follicles reach the right size, an injection will be administered to mature your eggs and you can collect them.

Stage 2

A small amount of sedation will be administered to you on the day of egg collection. To collect eggs, a fine needle connected to a scanning probe will be passed into your vagina. Your male partner will also need to provide a semen sample or, if using donor sperm for this purpose, it will need to be thawed.

Stage 3

After the eggs are collected, the sperm will be injected into the eggs to fertilize them. If there is a male factor issue or if the sperm is of poor quality on the day, the embryologist may suggest you use ICSI in which the sperm is injected directly into the egg to aid fertilization.

Stage 4

An embryologist will contact you the day after egg collection to discuss fertilization and the number of embryos. In most circumstances, embryos are cultured in the lab for five to six days after egg collection until they reach the blastocyst stage. Depending on your treatment plan, the embryologist will either freeze all your embryos for a FET ( Frozen Embryo Transfer) in a later cycle or advise you when your fresh embryo transfer will take place. The fluid inside embryos is removed before they are frozen in liquid nitrogen. The success rate of freezing embryos in vitrification has improved to the point that they are as effective as IVF cycles.

Using frozen embryos

The option to replace thawed embryos is between a natural (without drugs) and a primed cycle with hormone supplements. Your medical history and age will determine which treatment is best for you.

Fresh embryos

A fresh embryo transfer usually takes place between five and six days after egg collection, once the embryo has reached the blastocyst phase. To avoid multiple pregnancies, most women will only transfer one embryo. The embryo transfer procedure is simple. A speculum is inserted in your vagina, similar to a blood test. Once the embryo is inside the uterus, a catheter will be inserted.

Reserve embryos

You can freeze any good-quality embryos that you have left from your treatment to be used for future treatment or for a sibling. Your fertility specialist will arrange a Frozen Embryo Transplant when you are ready to use the embryos.

IVF pregnancy

You will have to wait between 12 and 14 days after the embryo transfer to determine if your treatment was successful. This two-week wait can prove to be very stressful for both couples and women. There is always anxiety about the outcome of treatment. The London Women’s Clinic offers counseling and support during treatment and hosts a monthly support group that allows patients to connect with others who are in similar situations. If your treatment fails, we will schedule a review appointment for you to discuss your cycle as well as any possible next steps. If you require support or counseling, we will be happy to serve you again
