Let’s Discuss…

How IVF is Performed at the Fertility World Toronto?

IVF is a series of procedures in which we retrieve eggs from a woman’s ovaries to be fertilized by sperm in our fertility laboratory, outside the body. Once the egg is fertilized, the resulting embryo is incubated in our laboratory for a few days under the culture medium for complete development before being transferred back into the mother’s uterus (womb).

Before transferring the embryo into the uterus, between fertilization and implantation, we can test the embryos for genetic competence using PGD or PGS. This ensures the highest level of success possible and minimizes the chances of twins or triplets, and also regulates if there are any genetic abnormalities in the embryo. In total, this treatment can consume two to four weeks, the length of an ovulation cycle. Each patient is unique, and the team at Fertility World fertility Centre works with you to develop a treatment plan that prioritizes your preferences and goals.

Understanding the Process of IVF Treatment:

Fertility World strongly recommends any fertility patient be aware of the basics of each phase to ensure the best results. Your experience with IVF treatments will go as follows:

  • Initial Consultation: After you book the first appointment with us, our coordinators will schedule a direct free consultation with our fertility specialist Doctor. They will get to know you and your medical history in detail. Based on your fertility history, we can best design an IVF treatment plan that works for you. We’ll also carry out preliminary tests to check on hormone levels and other factors.
  • Ovarian Stimulation: To boost your egg production for ovulation, fertility drugs (Follicle-stimulating hormone, Luteinizing hormone) can be used. Our fertility specialist will continually monitor these treatments for effectiveness. You will be given HCG (human chorionic gonadotropin) injection to allow for the maturation of the eggs.
  • Egg Retrieval: About 36 hours after the injection, an outpatient surgery called a vaginal ultrasound is performed for egg retrieval. This surgery is minor, and you can leave after an hour. Your partner will provide a semen (sperm) sample during this process. Sperm can be collected with masturbation, PESA, TESA, and TESE methods.
  • Embryo Development: The semen sample fertilizes the eggs and transfers them to incubators. The embryos are left to develop for the next few days. Fertilization can be achieved with conventional methods, AI, and ICSI treatments.
  • Embryo Selection: The embryo development is monitored for about five days. Then, we identify which embryos are the healthiest and least likely to carry a genetic disorder. The best blastocyst stages embryo is picked.
  • Embryo Transfer: We typically try single embryo transfer for most couples to avoid multiple pregnancies. An embryo is implanted using a catheter. Again, you’ll be able to go back to normal activities, as usual, the day after the embryo transfer procedure.
  • Post-Transfer: Our patients are prescribed to continue progesterone therapy for at least two weeks after the embryo transfer to make implantation easier. In the following days, we’ll monitor you for pregnancy symptoms. If your pregnancy is successful, you’ll continue progesterone therapy for another few weeks. We schedule a place for your delivery at our center. A healthy baby is Born every 2 seconds across the world.

Understanding the Surrogacy Programs, the Fertility World…

The greatest gift anyone can ever receive from someone (surrogate mother) is the Gift Of Life- BABY!. Surrogacy is an arrangement, contract often supported by a legal agreement, whereby a woman (the surrogate mother) agrees to bear a child for another person or persons, who becomes the child’s biological parent(s) after birth.

Surrogacy in Toronto (Canada) is a legal and highly successful alternative for intended couples, LGBT communities, single parents who have had difficulty or need assistance creating a family of their own. This journey should be well researched and prepared for. It can be a very rewarding experience for surrogate mothers and is often the only option for an infertile couple to have their own genetically related child.

Steps involved in Surrogacy…

  1. Decide if Surrogacy is the Right Choice- Becoming a surrogate or a parent through surrogacy can be a long and emotional journey, and it is a big commitment for both parties. Therefore before going for it, decide whether Surrogacy is right for you. There are several reasons to consider growing a family via surrogacy, whether you are a couple who has struggled with infertility, a member of the LGBT community, or are looking to expand your family as a single parent.
  1. Prepare for Surrogacy: Once you’ve decided to commit to surrogacy, one must determine the goals and needs of the surrogacy and the type of surrogacy professional you want to work with.

Fertility World specializes in Gestational Surrogacy with 100% success rates– Here, the child is not biologically related to the surrogate mother. The embryo is instead created using an egg from the intended mother or a donor and sperm from the intended father or a donor using in vitro fertilization.

  1. Finding a match between Surrogates and intended parents, or Donors if required: The most exciting and important step of the surrogacy process is finding the right surrogacy opportunity with a surrogate mother or intended parents.

When a prospective surrogate mother or intended parents begin working with a surrogacy professional, our surrogacy specialist will help develop your surrogacy plans. Based on the surrogacy plans, your profile is created to show to other intended parents or surrogate mothers who are also looking for a surrogacy opportunity. The matching is created officially by drafting the initial legal contract in the presence of a legal attorney.

  1. Satisfy the legal requirements: While drafting a legal contract. Each of you will have an attorney to ensure that your legal interests are represented and protected.

Once everyone agrees to the terms of the contract and each lawyer has had a chance to review and approve it, the contract is signed, and the embryo transfer process begins.

  1. Begins the Fertilization and Embryo Transfer Process: Surrogacy is incomplete without IVF procedures. Where partner sperm or donor sperm fertilizes the female partner’s egg or the donor eggs. All the IVF steps are followed, and the resultant embryo is transferred into the surrogate (Carrier) uterus to carry the pregnancy for the intended parents.
  1. Welcome your biological Baby delivered by the surrogate: After the long tiring surrogacy process, comes the birth of the baby! A life-changing event for both the surrogate and the intended parents. After the baby is born and the surrogate is discharged from the hospital, the new family and surrogate return home, the parents with their new baby and the surrogate with the satisfaction of giving the selfless gift of parenthood to someone who couldn’t do it on their own.

Some of the Fertility World Advanced fertility treatments are discussed below

Intrauterine Insemination (IUI): This treatment aims to increase the number of sperms that reach the fallopian tube and subsequently increase the chance of fertilization. IUI essentially provides sperm with head-start advantages, but still requires the sperm to reach and fertilize the egg on its own. This fertility treatment is performed when males have low sperm mobility and are weak. This is an alternative method before going for IVF. However, it depends on your unexplained fertility conditions.

Genetic Testing: This testing is done during an IVF cycle. Preimplantation genetic testing allows our specialist and laboratory team to identify genetic or chromosomal defects in an embryo before being used in an IVF cycle. This results in high-quality, chromosomally normal embryos. It assures the best chance of implanting successfully in the uterus and later developing into a healthy baby. The Fertility World in Toronto offers two different types of genetic tests for embryo evaluation: preimplantation genetic screening (PGS) and preimplantation genetic diagnosis (PGD).

Donor Egg Programs: The reproductive potential of some women is compromised because they do not produce eggs, produce low-grade eggs and, or embryos, or are carriers of a genetic condition. An option for these women is to undergo donor egg IVF, or surrogacy, that is done in conjunction with IVF treatment. Treatment with egg donation involves a woman who serves as an egg donor and a woman who serves as the recipient. Donor Egg IVF can be used for women who have a healthy uterus, but who either have no eggs (women with early menopause or history of surgical removal) or eggs that have not responded to medications or IVF.

Egg Freezing program: Some couples always want to have children but want to focus on their careers for the time being, or they are not financially stable enough to raise a child yet. Therefore, the Fertility World in Toronto offers the best alternatives option for mothers, fathers, couples with egg-freezing facilities for future use. Because delaying pregnancy can mean your eggs become older and less effective as you grow older. Your biological clock might not line up with your timeline. We bring you a new technology that makes it possible to freeze your eggs early and preserve them when you are ready to have children.

Fertility Preservation(embryo cryopreservation):

Almost 50% of patients left out unused extra embryos following their IVF cycle. These embryos can be saved for future use, providing the opportunity for additional attempts at pregnancy from a single IVF cycle. Most women consider freezing their embryos for social preferences, or for cancer fertility reasons (before receiving radiation or chemotherapy treatments which may damage their eggs). Only good-quality embryos are frozen as poorer quality embryos do not survive the freezing thaw cycle.

Intracytoplasmic sperm injection(ICSI): This is a common technique used in the process of in-vitro fertilization that takes IVF one step further.

ICSI involves a single sperm injected directly into the egg zona pellucida allowing the sperm to bypass several steps in the fertilization process. Instead of conception in a petri dish, this process involves injecting a single sperm directly into an egg. We select individual sperm from a semen sample. Once the sperm cell is selected, based on its normal head shape, good motility, and overall performance, the sperm cell is then inserted into an egg to fertilize.

ICSI is used to overcome male and female fertility issues, such as:

  • Low sperm to perform IVF or IUI.
  • Weak sperm motility
  • Sperm may have trouble attaching to the egg.
  • Blockage in the male reproductive tract.
  • Fertilization through IVF was not possible.
  • When matured eggs are used.
  • For using previously frozen eggs.

PESA/TESA/TESE: Testicular sperm extraction (TESE) is a procedure done to collect sperm directly from the testes when there is no sperm found in semen. This procedure is done when: Testes are not making enough sperm in the semen. A blockage has prevented the sperm from passing out of the penis. Men may have this procedure done to freeze their sperm before cancer treatment or to have a biological child after cancer treatment.

Testicular sperm aspiration (TESA) is performed by inserting a needle in the testis and aspirating tissue with no pressure.

Percutaneous epididymal sperm aspiration (PESA): A small needle is allowed to pass directly in the head of the epididymis and the fluid is aspirated. It does not require surgical incision and fertility treatments like Intracytoplasmic Sperm Injection (ICSI) use these extracted sperm cells.
