How IVF is Performed at the Fertility World Toronto?

IVF is a series of procedures in which we retrieve eggs from a woman’s ovaries to be fertilized by sperm in our fertility laboratory, outside the body. Once the egg is fertilized, the resulting embryo is incubated in our laboratory for a few days under the culture medium for complete development before being transferred back into the mother’s uterus (womb). 

Before transferring the embryo into the uterus, between fertilization and implantation, we can test the embryos for genetic competence using PGD or PGS. This ensures the highest level of success possible and minimizes the chances of twins or triplets, and also regulates if there are any genetic abnormalities in the embryo. In total, this treatment can consume two to four weeks, the length of an ovulation cycle. Each patient is unique, and the team at Fertility World fertility Centre works with you to develop a treatment plan that prioritizes your preferences and goals.  

Understanding the Process of IVF Treatment:

Fertility World strongly recommends any fertility patient be aware of the basics of each phase to ensure the best results. Your experience with IVF treatments will go as follows:

  • Initial Consultation: After you book the first appointment with us, our coordinators will schedule a direct free consultation with our fertility specialist Doctor. They will get to know you and your medical history in detail. Based on your fertility history, we can best design an IVF treatment plan that works for you. We’ll also carry out preliminary tests to check on hormone levels and other factors.  
  • Ovarian Stimulation: To boost your egg production for ovulation, fertility drugs (Follicle-stimulating hormone, Luteinizing hormone) can be used. Our fertility specialist will continually monitor these treatments for effectiveness. You will be given HCG (human chorionic gonadotropin) injection to allow for the maturation of the eggs.
  • Egg Retrieval: About 36 hours after the injection, an outpatient surgery called a vaginal ultrasound is performed for egg retrieval. This surgery is minor, and you can leave after an hour. Your partner will provide a semen (sperm) sample during this process. Sperm can be collected with masturbation, PESA, TESA, and TESE methods.
  • Embryo Development: The semen sample fertilizes the eggs and transfers them to incubators. The embryos are left to develop for the next few days. Fertilization can be achieved with conventional methods, AI, and ICSI treatments.
  • Embryo Selection: The embryo development is monitored for about five days. Then, we identify which embryos are the healthiest and least likely to carry a genetic disorder. The best blastocyst stages embryo is picked.
  • Embryo Transfer: We typically try single embryo transfer for most couples to avoid multiple pregnancies. An embryo is implanted using a catheter. Again, you’ll be able to go back to normal activities, as usual, the day after the embryo transfer procedure.
  • Post-Transfer: Our patients are prescribed to continue progesterone therapy for at least two weeks after the embryo transfer to make implantation easier. In the following days, we’ll monitor you for pregnancy symptoms. If your pregnancy is successful, you’ll continue progesterone therapy for another few weeks. Intrauterine Insemination (IUI): This treatment aims to increase the number of sperms that reach the fallopian tube and subsequently increase the chance of fertilization. IUI essentially provides sperm with head-start advantages, but still requires the sperm to reach and fertilize the egg on its own. This fertility treatment is performed when males have low sperm mobility and are weak. This is an alternative method before going for IVF. However, it depends on your unexplained fertility conditions.

Genetic Testing: This testing is done during an IVF cycle. Preimplantation genetic testing allows our specialist and laboratory team to identify genetic or chromosomal defects in an embryo before being used in an IVF cycle. This results in high-quality, chromosomally normal embryos. It assures the best chance of implanting successfully in the uterus and later developing into a healthy baby. Intracytoplasmic sperm injection(ICSI): This is a common technique used in the process of in-vitro fertilization that takes IVF one step further.

ICSI involves a single sperm injected directly into the egg zona pellucida allowing the sperm to bypass several steps in the fertilization process. Instead of conception in a petri dish, this process involves injecting a single sperm directly into an egg. We select individual sperm from a semen sample. Once the sperm cell is selected, based on its normal head shape, good motility, and overall performance, the sperm cell is then inserted into an egg to fertilize.  

ICSI is used to overcome male and female fertility issues, such as:

  • Low sperm to perform IVF or IUI.
  • Weak sperm motility
  • Sperm may have trouble attaching to the egg.
  • Blockage in the male reproductive tract.
  • Fertilization through IVF was not possible.
  • When matured eggs are used.
  • For using previously frozen eggs.  

PESA/TESA/TESE: Testicular sperm extraction (TESE) is a procedure done to collect sperm directly from the testes when there is no sperm found in semen. This procedure is done when: Testes are not making enough sperm in the semen. A blockage has prevented the sperm from passing out of the penis. Men may have this procedure done to freeze their sperm before cancer treatment or to have a biological child after cancer treatment.

Testicular sperm aspiration (TESA) is performed by inserting a needle in the testis and aspirating tissue with no pressure.

Percutaneous epididymal sperm aspiration (PESA): A small needle is allowed to pass directly in the head of the epididymis and the fluid is aspirated. It does not require surgical incision and fertility treatments like Intracytoplasmic Sperm Injection (ICSI) use these extracted sperm cells.
