What is In-Vitro Fertilisation (IVF)?
In vitro fertilization is a medical procedure or process of fertilization whereby an egg is combined with sperm in vitro (in a test tube or elsewhere outside the body in the fertility laboratory). In vitro fertilization (IVF) is a treatment for infertility or genetic problems.
Who needs IVF treatment?
You should consider IVF if you and your partners are encountering such problems mentioned below:
- If you and your partner are unable to get pregnant after long periods of trying intercourse.
- If you have failed after trying less-invasive treatments before attempting IVF.
- If you have failed after using fertility drugs to increase the production of eggs or intrauterine insemination.
- Advanced maternal age: Age is one of the single most important factors affecting pregnancy. Aging slows down the forming capacity and egg quality starts declining rapidly after 35 years of age. Also, chances of genetic abnormalities increase with increasing age. In these patients, IVF helps them to conceive and also enables them to screen embryos for any genetic abnormality.
- Couples who want to postpone pregnancy: If you want to focus more on your career in this era of the corporate world and competition. But as age advances the egg forming capacity in women decreases and so as the chances of conceiving naturally. These women can undergo IVF stimulation and can get their eggs or embryos frozen for future use. So that they can plan their pregnancy whenever they are prepared for it.
Hence IVF relieves their stress by bridging the gap which was essential for them to build up their career by making their wish come true at their ease.
- Unexplained infertility: Among the couples who are seeking infertility treatment, about 5% of couples are these couples whose reports remain normal but still they are unable to conceive. IVF is a direct key to their luck for conceiving.
IVF treatments can be your answer if you have certain health conditions. For example, IVF in Delhi may be an option if you or your partner has the following conditions:
- Fallopian tube damage or blockage: This tube damage or blockage makes it difficult for an egg to be fertilized or an embryo to pass to the uterus.
- Ovulation disorders: If ovulation is irregular or absent, only fewer eggs are possible for fertilization.
- Endometriosis: It occurs when the uterine tissue implants grow outside the uterus which usually affects the function of the ovaries, uterus, and fallopian tubes.
- Uterine fibroids: Benign tumors in the wall of the uterus which are common in women in their 30s and 40s. It can interfere with the implantation of the fertilized egg.
- Previous tubal sterilization or removal: If you had a tubal ligation (fallopian tubes are cut or blocked) to permanently prevent pregnancy but wish to conceive, IVF is an alternative to tubal ligation reversal.
- Impaired sperm production or function: Below-average sperm concentration, weak movement of sperm (poor mobility), or abnormalities in sperm size and shape can make it difficult for sperm to fertilize an egg.
- A genetic disorder: If you or your partner is at risk of passing on a genetic disorder to your child. The fertilized embryo is screened for certain genetic problems, although not all genetic problems can be found. Embryos that don’t contain identified problems are implanted into the uterus.
- Fertility preservation for cancer or other health conditions; If you’re a cancer patient before treatment such as radiation or chemotherapy IVF for fertility preservation is an option. Women’s eggs are harvested from the ovaries and frozen in an unfertilized state for later use. Or the eggs can be fertilized and frozen as embryos for future use.
Women who don’t have a functional uterus or for whom pregnancy poses a serious health risk might choose IVF using another person to carry the pregnancy (gestational carrier) known as surrogacy. In this case, the woman’s eggs are fertilized with sperm, but the resulting embryos are placed in the gestational carrier’s uterus.
How is IVF performed
IVF has been around for decades and most likely you know the basic idea behind IVF treatments. IVF is performed in the following steps respectively:
1. Ovulation induction: Generally it requires 1 to 2 weeks for ovarian stimulation. If you’re using your eggs during IVF, You’ll be given fertility drugs to stimulate your ovaries to produce multiple eggs. These drugs which contain Follicle Stimulating Hormone tell your body to produce more than just the normal one egg per month. The more eggs you produce, the more chances you’ll have of successful fertilization. Your ovaries are monitored using ultrasound regularly and check your hormone levels.
Also, You may require several different medications, such as:
- Medications for ovarian stimulation: Injectable follicle-stimulating hormone (FSH), a luteinizing hormone (LH), or both. These medications stimulate more than one egg to develop at a time.
- Medications for oocyte maturation: At the time of When the follicles are ready for egg retrieval, usually after eight to 14 days. Embryologists can suggest you take human chorionic gonadotropin (HCG) or other medications to help the eggs mature completely.
- Medications to prevent premature ovulation: It will prevent your body from releasing the developing eggs too soon.
- Medications to prepare the lining of the uterus: On the day of egg retrieval or at the time of embryo transfer, doctors can recommend you to start taking progesterone supplements to make the lining of the uterus more receptive to implantation.
Our professional doctors will work with you to determine which medications to use and when to use them.
2. Egg retrieval: Egg retrieval is done 34 to 36 hours after the final injection and before ovulation. During egg retrieval, you’ll be sedated or prescribed pain medication. Transvaginal ultrasound aspiration or abdominal ultrasound method is used for egg retrieval whereby an ultrasound probe is inserted into your vagina to specify follicles- a thin needle is inserted into an ultrasound guide to go through the vagina and into the follicles to retrieve the eggs. The eggs are detached from the follicles through a needle connected to a suction device. Multiple eggs can be removed in about 20 minutes. Mature eggs are placed in a nutritive liquid (culture medium) and incubated.
3. Sperm retrieval: If you’re using your partner’s sperm, he’ll provide a fresh semen sample through masturbation on the morning of egg retrieval. Sometimes testicular aspiration is used for sperm retrieval. You can also use donor sperm. Sperms are separated from the semen fluid in the fertility laboratory.
4. Fertilization: Fertilization can be achieved using either one of the methods:
- A conventional method of insemination: In this method, healthy sperm and mature eggs are mixed and incubated overnight.
- Intracytoplasmic sperm injection (ICSI): In this method, a single healthy sperm is injected directly into each mature egg. ICSI is usually used when the quality of sperm is weak to penetrate the membrane of the egg.
- Assisted hatching: About 5 to 6 days after fertilization, an embryo “hatches” from its surrounding membrane (zona pellucida), allowing it to implant into the lining of the uterus.
If you’re an older woman over 40 of age or had multiple failed IVF attempts, assisted hatching is a useful option.
Assisted Hatching is a technique where a hole is made in the zona pellucida just before transfer to help the embryo hatch and implant.
- Preimplantation genetic testing: In this technique, the embryos are allowed to develop in the incubator until they reach a stage where a small sample can be removed and tested for specific genetic diseases or the correct number of chromosomes, typically after 5 to 6 days of development. Embryos that don’t contain affected genes or chromosomes are picked and transferred to your uterus. While preimplantation genetic testing can reduce the likelihood that a parent will pass on a genetic problem, it can’t eliminate the risk.
5. Embryo transfer: Embryo implantation usually takes place 2 to 5 days after egg retrieval. During the implantation procedure, You might be given a mild sedative. The procedure is usually painless, one can feel mild cramping.
The doctor will insert a long, thin, flexible tube called a catheter into your vagina, through your cervix, and into your uterus. A syringe containing one or more embryos suspended in a small amount of fluid is attached to the end of the catheter. Using the syringe, the doctor places the embryo or embryos into your uterus. If successful, an embryo will implant in the lining of your uterus about 6 to 10 days after egg retrieval.
After the successful embryo implantation inside the lining of the uterus, you can resume normal daily activities. However, your ovaries may still be enlarged. Avoid vigorous activity, which could cause discomfort.
What will you feel after successful implantation?
One could feel mild experiences after the implantation of the embryo:
- Passing a small amount of clear or bloody fluid shortly after the procedure
- Breast tenderness due to high estrogen levels
- Mild bloating
- Mild cramping
- Constipation
How do you know, you’re conceived?
After 12 days or 14 days of your egg retrieval, your blood sample is tested by your doctor to confirm whether you’re pregnant or not pregnant.
- If you’re pregnant: You will be sent to an obstetrician or other pregnancy specialist for prenatal care.
- If you’re not pregnant: Stop taking progesterone and likely get your period within a week. If you’re interested in attempting another cycle of in vitro fertilization (IVF).
What can you expect from IVF pregnancy delivery?
The privilege of giving birth to a healthy baby after using IVF depend on your various factors, including:
- Maternal age: The younger you are, the more likely you are to get pregnant and give birth to a healthy baby using your eggs during IVF. Women above 40 years and older are often counseled to consider using donor eggs during IVF to increase the chances of success rate.
- Embryo status: Transfer of more developed embryos is associated with higher pregnancy rates compared with less developed embryos. However, not all embryos survive the development process.
- Lifestyle factors impact: Women who smoke typically have fewer eggs retrieved during IVF and may miscarry more often. Smoking can lower a woman’s chance of success using IVF by 50%. Obesity can decrease your chances of getting pregnant and having a baby. The use of alcohol, recreational drugs, excessive caffeine, and certain medications also can be harmful.
