Understanding the Embryology

Embryology is the discipline concerned with the study of prenatal development of gametes (Eggs and Sperms), fertilization of the gametes, and development of embryos and fetuses. The study of embryology has helped in the understanding of congenital abnormalities and developing assisted reproduction procedures.

Who is an embryologist?

An embryologist is a scientist, highly trained medical professional who has studied embryology, having the expert skills of any clinical developments in the field of embryology. He/She specializes in the care of embryos from the time of egg retrieval to the time when the embryo is implanted into the woman’s uterus (Womb) to have a positive pregnancy. 

What are the responsibilities of the embryologist?

The embryologist is a key member of the fertility team and is responsible for:

  • Maintaining the lab environment for the healthy development of the embryos.
  • Create viable embryos to be either used in IVF or frozen for later IVF.
  • They ensure that the embryology lab is an ideal environment for embryo growth and storage.

Understand In-Vitro Fertilization

In vitro fertilization (IVF) is an assisted reproduction procedure that helps with fertilization, embryo development, and implantation, so you can get pregnant and deliver a healthy child. An embryologist plays a key role in IVF treatment.

How is the process of IVF?

The process of IVF involves a set of series procedures which is executed by skilled fertility professionals (Embryologist, Gynaecologist, Obstetricians, Nurses, etc). IVF works by using a combination of medicines and surgical procedures to help sperm fertilize an egg, and help the fertilized egg implant in your uterus (Wombs). It involves the following steps:

  1. Ovulation induction: Firstly, a fertility drug is given to help the ovaries to produce multiple eggs that are mature and ready for fertilization. Normally, females produce one egg per menstruation cycle.

To monitor the hormone levels and egg production an ultrasound or blood test is carried out often, till achieving its maturity.

  1. Egg Retrieval: When the egg reaches its maturity levels, embryologists retrieve the eggs from the ovary containing fluid-sac follicles. This procedure is a minor surgical process and is done at the doctor’s office. A medicine can be given before removal for you to relax and be comfortable during the process. An ultrasound guide is used and the doctor puts a thin, hollow tube through the vagina and into the ovary and follicles that hold your eggs. The needle is connected to a suction device that gently pulls the eggs out of each follicle.
  1. Fertilization: The eggs are mixed with sperm cells from your partner or a donor — called insemination. The eggs and sperm are kept under the culture medium for fertilization to occur. 

For sperm that have lower motility (don’t swim as well), they may be injected directly into the eggs to promote fertilization, a process called Intracytoplasmic sperm injection (ICSI). As the cells in the fertilized eggs divide and become embryos, people who work at the lab monitor the progress and achieve fertilization.

  1. Embryo Transfer: After 3-5 days of the egg retrieval, the best stage embryo is selected and transferred into the uterus for carrying a pregnancy. 

For the process, the doctor slides a thin tube through your cervix into your uterus and inserts the embryo directly into your uterus through the tube. An ultrasound is carried out regularly to confirm pregnancy. Embryo transfer is done at your doctor’s office or a fertility clinic, and it’s usually not painful.

What can be done after embryo transfer to ease the pain?

Plan on resting for the rest of the day after your embryo transfer. You can go back to your normal activities the next day. You may also take pills or get daily shots of a hormone called progesterone for the first 8-10 weeks after the embryo transfer. The hormones make it easier for the embryo to survive in your uterus.

Which people can undergo IVF treatment?

IVF is the only treatment when infertility is due to:

  • Blocked fallopian tubes or severe male factor infertility. 
  • Egg quality or ovulation problems
  • Endometriosis
  • Antibody issues, which harm eggs or sperms
  • Sperm count or motility issues
  • Unexplained infertility

People who have cancer also arrange to have their healthy eggs or sperm frozen, as chemotherapy and radiotherapy can harm your fertility. These eggs or sperm can be thawed later and used in IVF once the cancer treatment is over.

What is the best age for women, for a successful IVF?

  • The best reproductive age for women is between 21 to 35 years.
  • IVF is possible beyond 45 years, but the success rate is Uncertain unless you have preserved eggs.
  • Women who intend to use their eggs should be in their early 40s and up to 45 years.
  • Donor eggs, whether from known, identified, or anonymous donors, are eligible for women below the age of 50.
