The term In-Vitro Fertilization means fertilization of the egg and sperm outside the body, medically by growing them in Petri dishes under culture media inside the fertility laboratory, then implantation of the fertilized egg into the woman’s uterus (womb), completing within 3 weeks time. Let’s look at the detailed IVF process step by step: 

IVF Process step by step:

  1. Initial consultation: At fertilityworld, the patient is provided with free consultation. This step determines whether IVF can favor you or not.
  1. Ovulation: After the initial menstrual cycle, the ovulation or superovulation (release of the egg from the ovary) begins by giving fertility medication/IVF injections including FSH or LH  hormones. These hormones boost the body and enhance multiple egg production than normal. The more eggs produced, the more chances of successful fertilization later in the treatment. Transvaginal ultrasounds and blood tests are done regularly during this step, it checks ovaries and monitors the hormone levels.
  1. Trigger Injection: After successful ovulation, the right time to administer the trigger injection (hCG) is scheduled, this will aid the egg’s final maturation and loosen it from the follicle wall. This trigger injection-specific time is crucial.
  1. Egg retrieval/pickup: The egg retrieval takes place on the 2nd morning after the final trigger injection (34-36 hours later). The embryologist will perform a minor surgical procedure known as the follicular aspiration to retrieve the eggs or “egg-pick up” under very light sedation. An ultrasound-guided probe guides the fine needle into each ovary through the vaginal canal. This fine needle attached to the suction mechanics sucks out the egg, one at a time. The fluid in the follicle is then extracted into a test tube and studied under a microscope to look for eggs. This procedure takes about 20 to 30 minutes, depending on the number of follicles that have developed. This step is performed as an outpatient.
  1. Semen Sample Collection: While the eggs are being retrieved, a male partner is asked to produce his semen sample through traditional methods or via surgical sperm aspirations. It may be possible to produce the sample at home and drop it off at the clinic, or freeze his semen samples as a backup. Patients can also choose to use donor sperm (if required). The sperm are then put through a high-speed wash and spin cycle to find the healthiest ones.
  1. Fertilization: The best-concentrated sperm is combined with the retrieved eggs and cultured in Petri dishes under the culture medium called insemination. Then, the eggs are kept in an incubator overnight and examined the next day for signs of fertilization. Mostly, 60-70% of eggs get fertilized, if the sperm sample appears normal. The fertilized eggs are then kept in the incubator for 48 hours for complete mature development. In some cases, IVF specialists may suggest an ICSI procedure, if the sperm quality is low (low sperm count, morphology, or motility). ICSI is a specialized form of treatment for male infertility that involves injecting a single sperm directly into a single mature egg.
  1. Embryo Transfer: The transferring of the embryo begins about 2-5 days after fertilization. The chosen best-stage embryo is placed into a thin plastic catheter (a soft tube), that passes through the cervix, into the uterus, and gently releases the embryo which is known as the IVF implantation process in India. Generally, Multiple embryos are transferred in the hopes that at least one will implant itself in the lining of the uterus and begin to develop. Sometimes more than one embryo ends up implanting, which is why multiples are possible in women who use IVF. The process itself takes only a few minutes and is usually painless. This part of the IVF process is performed in the doctor’s office while you are awake. After the transfer, some of the remaining embryos may be suitable for vitrification (rapid freezing), and this option is available to all patients undergoing IVF treatment. Frozen embryos can be used in subsequent cycles if the first one is not successful. 
  1. The luteal phase: This is the phase/time between the embryo transfer and the pregnancy test. During this time, the patient must look after herself and maintain good health and well-being. The IVF process replicates natural reproduction.
